Procedure for Prostate

Dr. Suntchai Wirotsaengthong

Urological surgeon at BNH hospital,

Bangkok, Thailand

Specialising in

  • Minimally invasive procedure for prostate

BPH is a non-cancerous enlargement of the prostate gland that can cause many urinary symptoms. Minimally invasive surgery for Benign Prostatic Hyperplasia (MIS for BPH) involves techniques that aim to treat an enlarged prostate with less disruption to surrounding tissues compared to traditional surgery.

  • Prostate cancer detection by MRI fusion prostate biopsy

MRI fusion prostate biopsy is an advanced technique used in the diagnosis and evaluation of prostate cancer. This procedure combines the information from a magnetic resonance imaging (MRI) scan of the prostate with real-time ultrasound images during a biopsy. The goal is to improve the accuracy of prostate cancer detection and enhance the ability to target suspicious areas within the prostate. Dr Suntchai started doing MRI fusion prostate biopsy in 2017.

  • Urinary tract stone treatment

Treatment by either simple endoscope of urinary system such as Ureteroscopy or using Retrograde intrarenal surgery (RIRS) which is a minimally invasive surgical procedure by using flexible ureteroscope to access upper urinary tract and kidney for the treatment of kidney stones and other conditions affecting the upper urinary tract. This technique allows urologists to access and treat stones or other abnormalities inside the kidney without the need for an incision.

  • Vasectomy

Non Scalpel vasectomy is our prefer technique without need of stitch after the procedure. This could be done under local or sedation

Water vapor therapy

What is Rezum?

REZUM, Water Vapor Therapy

Is water vapor therapy right for you

Rezum, Water vapor therapy is recommended to: • men with BPH but can not or do not want to take oral medicines. • men with chronic medical conditions that prevent  them from having general anesthesia. In order to determine whether Water Vapor Therapy is the most appropriate option for your case, a consultation with urologists is suggested.

The Rezum system uses water vapor therapy to treat BPH. The procedure involves injecting steam into the prostate tissue, causing the immediate cell death and the prostate to shrink over time.

During the Rezum procedure, a small device is inserted into the urethra, and the steam is delivered via the small needle to the prostate tissue. The number of needle treatments depends on how big your prostate is and the whole treatment may take only 10-15 minutes. This targeted application of steam helps to reduce the size of the prostate, relieving pressure on the urethra and improving urinary symptoms such as frequent urination, urgency, and difficulty starting or maintaining a steady stream.

Rezum is considered a minimally invasive treatment option for BPH and is often performed on an outpatient basis. It is an alternative to more traditional treatments for BPH, such as medication or surgery.

Why Choose Water Vapor Therapy ?

Convenience

  1. outpatient, same-day procedure

ITind
 Reshaping Prostate

What is iTind?

iTind Reshaping Prostate

iTind is the newest minimally invasive treatment for BPH.
By using temporary nitinol device, it will reshape the prostatic urethra, gradually creating a wider opening through for urine to flow freely.
With iTind, lower urinary tract symptoms due to an enlarged prostate will be relieved without burning or cutting out tissue, and without leaving behind a permanent implant.

The insertion of iTind

  1. The insertion will be performed as outpatient procedure in operating room by a urologist. A small thin tube with a tiny camera will be inserted into the prostatic urethra to determine where to place the iTind.
Once the device is put, patient is able to urinate freely and can go home after that.
  2. Light sedation or a local anesthetic may be given.

During 5-7 Days

  1. iTind device is specifically designed as folded configuration.
  2. During 5-7 day-treatment, it helps to create gently pressure on the prostate, open up the prostatic urethra  in order to clear a way for the flow of urine.

The removal of iTind

  1. After 5-7 days, the urologist will use a flexible silicone catheter to remove the iTind device completely.
  2. Patient will return to normal activities 1-2 days after the removal.

Who is suitable for the iTind?

iTind is the best treatment option for:

  •    –     Mild to moderate BPH case with a prostate volume ≤ 75ml.
  •    –     Men with BPH but do not want to treat with oral medicine or can not bear the side effects of medicines.
  •    –     Who want to preserve sexual function (retrograde ejaculation or erectile dysfunction).

* Men with an obstructive median lobe or previous prostate surgery are not the right candidates for iTind.

Urolift

Minimally invasive medical procedure

What is Urolift?

UroLift is a minimally invasive medical procedure used to treat benign prostatic hyperplasia (BPH), commonly known as an enlarged prostate. It involves the use of a small device to lift and hold the enlarged prostate tissue out of the way of the urethra, which helps improve urine flow. This procedure is typically performed in a doctor’s office or clinic with local anesthesia, and it does not require cutting or removal of prostate tissue, making it a gentler alternative to traditional prostate surgeries like TURP (Transurethral Resection of the Prostate). This innovative approach provides relief from bothersome symptoms like frequent urination or weak urine flow.

How the UroLift System Works

The UroLift procedure is a minimally invasive, same-day outpatient treatment that can even be performed in a doctor’s office under local anesthesia. Most patients can return home the same day without requiring an overnight stay or a catheter after the procedure.

If you and your healthcare provider decide that the UroLift System is the right treatment for you, your doctor will provide you with detailed information about the procedure.

Overall, the UroLift System is a safe and effective alternative for individuals seeking a less invasive solution for managing BPH symptoms. This innovative procedure offers a shorter recovery time compared to traditional surgical treatments. Patients typically experience minimal discomfort and rapid relief from urinary symptoms, allowing for a quicker return to daily activities and an improved quality of life.

Step of Urolift

  1. Insertion of Delivery Device: A urologist inserts a small delivery device through the urethra to access the prostate.
  2. Placement of Implants: Tiny permanent implants (like small clips) are placed on both sides of the prostate. These implants hold the enlarged prostate tissue away from the urethra, creating a wider passage for urine flow.
  3. Immediate Relief: Unlike other procedures that remove or destroy prostate tissue, UroLift simply repositions the tissue, allowing for immediate symptom relief without affecting sexual function.
  4. Quick Recovery: The procedure is typically done in an outpatient setting, often requiring only local anesthesia, and most patients return to normal activities within a few days.

It’s often recommended for men experiencing moderate to severe symptoms of BPH who want a faster recovery and fewer risks compared to more invasive surgical option

Why choose Urolift

  • Rapid recovery with minimal downtime
  • Preservation of sexual function
  • No need for incisions or cutting of the prostate
  • Quick improvement in symptoms like frequent urination, urgency, and weak urine flow

Who Is a Suitable Candidate for UroLift?

If you are a male aged 50 or older experiencing symptoms of Benign Prostatic Hyperplasia (BPH), you may be a good candidate for the UroLift System.

We recommend consulting with your urologist to determine if this treatment is right for you. It is important to inform your healthcare provider if you have allergies to nickel, titanium, or stainless steel before undergoing the procedure.

The UroLift System is most effective for individuals with moderate to large prostate glands (not exceeding 100 grams in size) and those who are still able to urinate or have less than 350 cc of post-void residual volume.

Urolift Frequently Asked Questions.

The implants are designed to be permanent in nature. They are composed of standard surgical implantable materials, namely a nitinol capsular tab, a stainless steel urethral tab, and a polyethylene terephthalate suture that securely fastens the two tabs together. In the event that removal of the urethral tab becomes necessary, your physician is able to do so by cutting the suture and extracting the tab with a conventional grasper tool. It is important to note that while the urethral tab can be removed, the capsular tab along with a portion of the suture will remain in place within your body.

As part of our commitment to patient safety, we provide patient implant cards to inform you that the UroLift implant is classified as MR Conditional. This means that the implant can safely undergo magnetic resonance imaging (MRI) scans under specific conditions. It is important to follow these specific MR conditions to ensure your safety and well-being during the scanning process. If you have any questions or concerns about the MRI compatibility of your UroLift implant, please do not hesitate to reach out to us for further clarification and guidance.
